Passport Requirements

  • Your passport must have been issued less than 15 years ago. Otherwise, review the requirements for a new passport. Other renewal eligibility requirements also apply. 
  • Provide your current original passport. 
  • Complete and sign Form DS-5504 if you meet its eligibility requirements.  DS-5504
  • Complete and sign the letter of authorization.  Download form
  • Provide evidence of your legal name change, such as a certified marriage certificate, divorce decree, court order, or other government-issued document showing your new name.
