Passport Requirements

  • Visit a passport acceptance facility, such as a participating post office or clerk of court, to have your documents executed and sealed. An appointment may be required. Tell the acceptance agent that a courier will hand-carry the application. Send Duke’s the sealed envelope without opening it.  Local Acceptance Facility
  • Complete and print one Form DS-11 using the online form filler, including its 2D barcode. Do not sign it until instructed by the acceptance agent.  Form DS-11
  • Provide original or certified evidence of U.S. citizenship, such as a U.S. birth certificate, Certificate of Citizenship, or Certificate of Naturalization. 
  • Provide acceptable photo identification. A mutilated, altered, or damaged passport is not acceptable as proof of identity. Examples of other identification include a current, valid driver’s license, government ID, or military ID. 
  • Provide two passport photos. Review the photo requirements before submitting them.  Photo requirements
  • Optional: Include an itinerary, round-trip airline tickets, or a business letter on official letterhead addressed to the Passport Agency stating your travel dates and purpose of travel. 
  • Complete and sign the letter of authorization.  Download form
  • The child and parents or guardians generally appear in person. If one parent cannot attend, review the consent requirements for Form DS-3053 and any applicable exceptions.  Form DS-3053
  • If an absent parent cannot be located, review the special family circumstances instructions for Form DS-5525 and provide the required supporting evidence.  Form DS-5525
  • Provide a check or money order for $186.87 or $201.87 with a passport card payable to the U.S. Department of State. 
  • Include a separate set of copies outside the sealed envelope for Duke’s review. An unsigned copy of Form DS-11 is acceptable for this review. 
  • Processing days exclude weekends and holidays. 
  • If you previously received a passport card and are requesting a replacement or renewal, include your existing card as directed.
