Passport Requirements

  • Your passport must have been issued less than 15 years ago. Otherwise, review the requirements for a new passport. Other renewal eligibility requirements also apply.  New passport requirements
  • Complete and print one Form DS-82 using the online form filler, including its 2D barcode.  Form DS-82
  • Provide two passport photos. Review the photo requirements before submitting them.  Photo requirements
  • Optional: Include an itinerary, round-trip airline tickets, or a business letter on official letterhead addressed to the Passport Agency stating your travel dates and purpose of travel. 
  • Complete and sign the letter of authorization.  Download form
  • If your name has changed, include the original marriage certificate or other legal name-change document and a copy of your driver’s license. 
  • Print the order form generated when you complete your Duke’s order. 
  • Processing days exclude weekends and holidays. 
  • If you previously received a passport card and are requesting a replacement or renewal, include your existing card as directed.
